CYL - Application Forms

  1. New for 2010: Online participant application forms.  Filling out the online application will take about 15-20 minutes and requires that you have a knowledge of the participant's health.
  2. The PDF Application Forms can be filled out while online, or can be printed off and filled in later.  These are the same forms that were included in the CYL 2010 printed newsletter.  
       The printed application form can be mailed, faxed or scanned/emailed to On Co-op at your convenience, as long as it - and the participant fee - reaches our offices by the due date.  This allows you to gather information, and return to the application many times.   
  3. Beginning in 2011, the only method for submitting participation applications will be online.


You may wish to quickly review the materials required to answer all of the questions (such as medical information) and have them close at hand when you complete the form online. The best way to do this is to quickly look at the fillable PDF version on the screen, exit and return when you have all of the information.

  1. Participants must have arranged to have a sponsor before applying.  Your completed application (including the participant fee) must arrive at On Co-op by the due date.
  2. If you cannot find a sponsor, and have exhausted all of your leads, contact On Co-op. Before you contact us, check with any co-ops or credit unions you are members of, then check with your parents or other relatives who may be members, staff or volunteers of a co-op or credit union. Do NOT fill out an application form unless you have a sponsor.
  3. To take advantage of the early bird discount, your completed applications including participant payment must reach On Co-op by the due date.
  4. If you are a Facilitator, we strongly suggest you review the information contained in the Facilitator webpages including time commitments and other obligations.
  5. If you are an Intern, please review the Intern webpages and then contact On Co-op before beginning any of the applications.
  • Space permitting, you may apply for a CYL session up to 2 weeks before the start of each session, however sessions fill up very quickly and your attendance is not guaranteed!
